Aurora North Medical Centre provides referral-based services for physicians and nurse practitioners across Ontario.

Find below referral forms for our clinics and programs. Please complete the appropriate form and submit it according to the instructions listed on each document.

Cognitive Behavioural Therapy (CBT)

  • We are currently accepting referrals for Cognitive Behavioural Therapy.  

Service highlights:

  • OHIP covered

  • Virtual appointments

  • For patients 18+

  • Referral required from a family physician or nurse practitioner

  • Patients across Ontario are accepted

  • Current wait time: approximately 2–3 weeks

  • Visits do not impact access bonus for primary care models (FHO)

Chiropodist

Core Services:

  • Routine & Preventative Foot Care: Nail care, callus/corn management, skin and nail pathology, and full foot assessments
  • Diabetic & High-Risk Foot Care: Risk assessments, neuropathy screening, ulcer prevention, and ongoing management
  • Custom Orthotics & Biomechanics: Gait analysis and orthotic therapy for plantar fasciitis, flat feet, overuse injuries, and structural abnormalities
  • Foot & Ankle Pain Management: Diagnosis and treatment of heel pain, forefoot pain, tendonitis, and other musculoskeletal conditions
  • Nail Procedures & Minor Surgery: Management of ingrown/infected nails, including partial or total nail avulsion
  • Sports & Activity-Related Injuries: Assessment, treatment, and return-to-activity planning
 

Common Referrals Include:

  • Skin & nail conditions (calluses, corns, fungal nails, ingrown toenails)
  • Musculoskeletal issues (plantar fasciitis, Achilles tendonitis, bunions, neuromas)
  • Diabetic and high-risk foot concerns (neuropathy, ulcer prevention)
  • Gait abnormalities and biomechanical dysfunction
